- Shift: Monday to Friday | 9:00 am to 6:00 pm EST.
- Work setup: In-Office, Bogota/Medellin

- Responsible for a designated patient caseload, focusing on proactive prevention of hospital admissions.
- Conduct daily, weekly, or monthly follow-ups to ensure patients have access to and are adhering to prescribed medications.
- Schedule and verify patients' appointments, offering necessary assistance in securing timely care.
- Educate patients on their medications and wellness practices to keep them happy, healthy, and hospital-free.
- Lead and guide the team with your advanced medical knowledge.
- Provide clinical guidance and support to the nurse team.
- Demonstrate exceptional multitasking abilities to manage various aspects of patient care.
- Accurately input patient data into the company's medical system, maintaining up-to-date records.
- Specialize in the healthcare needs of geriatric populations, ensuring their ongoing well-being.
- Implement preventive measures, examinations, and medication adherence to promote and sustain health.
- Oversee and audit compliance with established protocols and information accuracy.
- Escalate medication changes or critical situations as necessary.
- Vigilantly assess patients' conditions to ensure hospitalization is only recommended when absolutely necessary.
- Skillfully filter cases to determine the appropriate course of action.
