Role Description

This full-time remote role is for a Bilingual RPM/CCM Virtual Assistant (English & Spanish) supporting healthcare providers and their patients. The Virtual Assistant will assist with Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) activities, including making outbound and inbound patient calls, documenting interactions in clinical systems, and following established care protocols. Daily tasks include communicating with patients in both English and Spanish, gathering health-related information, reinforcing care plans, and escalating concerns to clinical staff when needed. The role also involves scheduling follow-ups, updating records accurately, and coordinating with practice teams to ensure timely and consistent patient support.

Role Requirements
- Manage inbound and outbound phone calls,



providing professional and empathetic customer support in both English and Spanish.
- Schedule, confirm, reschedule, and coordinate patient appointments while maintaining accuracy and efficiency.
- Respond promptly to customer inquiries via phone and email, resolving issues or escalating when appropriate.
- Maintain accurate patient and client records, updating scheduling systems and documenting all interactions.
- Communicate with patients, healthcare providers, and employer group representatives to ensure smooth coordination of services.
- Assist members with general program information, appointment preparation, and follow-up communications.
- Monitor voicemail and referral email inboxes, ensuring timely responses and effective case management.
- Adhere to HIPAA, company policies, and confidentiality standards while delivering a high level of customer service.
