About the Role
We are seeking a Bilingual Patient Care Coordinator with healthcare administration or home health experience to support daily care coordination, client and caregiver communication, scheduling, documentation, and administrative workflows.
What You'll Do
Care Coordination & Client Management
- Monitor client schedules daily and ensure all open visits are assigned.
- Communicate with families regarding schedule changes, caregiver updates, and service interruptions.
- Track hospitalizations, discharges, and resumed services and maintain the hospitalization tracker.
- Coordinate with schedulers and leadership to resolve scheduling and client-care issues.
- Maintain accurate client profiles and service notes.
Caregiver & Scheduler Support
- Assist schedulers with confirming caregiver availability.
- Notify caregivers of schedule changes, cancellations, and new assignments.
- Ensure caregivers receive accurate visit instructions, including location, time,
and client needs.
- Track caregiver call-outs and escalate coverage risks promptly.
Documentation & Compliance
- Ensure required documentation is completed and uploaded, including visit notes, care plans, and hospitalization records.
- Verify Electronic Visit Verification (EVV) compliance and follow up on missing clock-ins and clock-outs.
- Maintain accurate, audit-ready records for Medicaid LTC clients.
- Support clinical staff by preparing client updates, visit summaries, and follow-up reminders.
Communication & Follow-Up
- Coordinate with RNs, HHAs, schedulers, and administrative staff to resolve issues.
- Monitor daily Asana task posts and track team completion.
- Prepare end-of-day summaries covering open visits, pending follow-ups, hospitalizations, and caregiver issues.
General Administrative Support
- Assist with onboarding activities, including document collection, reminders, and follow-ups
