Role Overview Our client is seeking a Remote Patient Care Coordinator (Cardiology Patient Navigator) to support a busy cardiology care team. The role centers on patient communication, scheduling, care coordination, and clinical documentation. The adecuado candidate brings a clinical background, experience in telephonic or remote care settings, and comfort managing a high-volume caseload of 150+ patients across multiple priorities. Cardiology-specific experience is preferred but not required. Candidates with strong backgrounds in care coordination, care management, med-surg, ICU,
