Role Overview

Our client is seeking a full-time, contract Clinical Pharmacist with strong cardiology experience to support value-based care programs. This role focuses on optimizing guideline-directed medical therapy (GDMT), supporting medication titration, improving medication adherence, and collaborating closely with nurses and cardiology providers to improve outcomes for high-risk cardiac patients.



This is not a dispensing role — the pharmacist functions as a clinical partner embedded within the care team.

Key Responsibilities

Medication Review & GDMT Optimization
- Review patient medication profiles for cardiology populations — heart failure, coronary artery disease, atrial fibrillation, hypertension, and overlapping chronic conditions such as CKD.
- Evaluate appropriateness of GDMT based on ACC/AHA guidelines and payer quality programs.
- Identify gaps in therapy, contraindications, drug interactions, and adherence barriers.
- Make evidence-based recommendations for medication initiation, optimization, and titration, in collaboration with providers and nursing staff.

Patient Engagement
- Conduct telephonic medication reviews and patient education.
- Educate patients on purpose, benefits, side effects, and expectations of cardiac medications.
- Reinforce adherence and lifestyle considerations; help patients recognize red-flag symptoms.
