About the Role

We are seeking a Bilingual Clinical Care Coordinator with a strong clinical background to remotely support a U.S.-based healthcare practice. This role is idóneo for a qualified healthcare professional who has transitioned into healthcare administration, care coordination, or virtual medical support.

The Clinical Care Coordinator will serve as an important link between patients, providers, insurance companies, and other members of the care team, helping ensure patients receive timely and well-coordinated care.

Key Responsibilities
- Communicate with patients regarding appointments, treatment plans, follow-ups, referrals, and other care needs.
- Coordinate patient care between providers, specialists, facilities, pharmacies, and other healthcare professionals.
- Assist with patient intake and maintain accurate patient information within the EHR/EMR.
- Document patient interactions, clinical information,



and provider instructions accurately.
- Coordinate and follow up on referrals and specialist appointments.
- Assist with insurance verification, eligibility checks, and prior authorizations.
- Follow up on outstanding medical records, laboratory results, imaging, referrals, and other clinical documentation.
- Conduct patient follow-up and care management calls, including Chronic Care Management (CCM) calls when required.
- Review patient information and identify issues that require escalation to the appropriate clinical provider.
- Communicate provider notes, instructions, and other relevant information to patients.
- Support prescription/refill requests and other clinical administrative workflows as appropriate.
- Maintain accurate, confidential, and HIPAA-compliant patient records.
- Provide compassionate and professional patient support while helping patients navigate their care journey.
