Gaudenzia, Inc. seeks a dedicated counselor to conduct intake interviews and orient clients to care coordination prior to discharge from residential care.
You will liaise with clinical staff to verify recommended services and coordinate referrals. You will develop Recovery Plans with clients, identify health care and recovery support needs, and help clients advance to the next level of care by removing barriers and conducting outreach.
