Job Title: Bilingual Patient Outreach & Care Coordination Specialist (English & Spanish)

Full-Time | 40 Hours per Week | Remote

We are looking for a full-time, bilingual (English and Spanish) Patient Outreach & Care Coordination Specialist for a Medicaid-focused behavioral health and peer support program.

Responsibilities:

- Call potential patients and explain the program, its benefits, and how it can help them in both English and Spanish.
- Clearly communicate the value of participating and encourage eligible patients to enroll.
- Answer questions, address concerns, and guide patients through the enrollment process.
- Help patients schedule appointments with therapists, peer support workers, and other providers.
- Follow up with patients to ensure they attend scheduled appointments.
- Contact patients who miss appointments and help them reschedule.
- Coordinate with therapists, peer support workers, and internal staff to ensure patients receive the services they need.
- Maintain consistent communication with patients to keep them engaged in the program.
- Track appointments, patient interactions, and follow-up activities.




- Ensure no patients fall through the cracks.

Requirements:

- Fluent in both English and Spanish (required), with excellent verbal communication skills in both languages.
- Friendly, empathetic, persuasive, and patient-focused personality.
- Experience in healthcare outreach, patient coordination, appointment setting, or customer service preferred.
- Strong organizational skills and ability to manage multiple patients and follow-ups.
- Comfortable making outbound calls throughout the day.
- Experience with U.S. healthcare or Medicaid programs is a plus.
- Ability to follow HIPAA privacy and security requirements.
