Are you a physician looking to step into a dynamic care coordination role with a focus on quality, patientcentered service Were seeking a Patient Care Coordinator PCC to work directly with healthcare providers and clinical staff, ensuring smooth communication, accurate treatment plans, and efficient care delivery.

As a PCC, you will

Collaborate closely with doctors, clinical teams, and administrative personnel.
Assist in updating and managing treatment plans as directed by the Primary Care Provider PCP, ensuring accurate coordination and followthrough.
Conduct interviews with patients and family members to complete accurate assessments.
Advocate for patients by providing education, guidance, and support throughout their care journey.
Prioritize and coordinate treatment activities in line with medical protocols.

Position Requirements

You must be a physician holding a valid medical degree. Other healthcare roles or nonmedical applicants will not be considered.




Must currently reside in Bucaramanga, Floridablanca, Girón, or Piedecuesta. Applications from candidates residing outside the Bucaramanga metropolitan area will not be considered.
Availability to complete the initial 23 month onsite training period in Bucaramanga before transitioning to remote work.
Strong computer literacy and digital fluency.
Intermediate to Advanced English proficiency written and spoken is essential.
